Enrollment is easy. Simply mail the applicable completed forms along with payment to
K&K Insurance, Attn: Leisure, P.O. Box 2338, Ft Wayne, IN 46801. Your coverage will
be effective the day after application & premium are received. You will then receive an
insurance packet consisting of a certificate of insurance evidencing coverage, as well
as other pertinent information.
