Application Package
Enrollment is easy. Simply mail the applicable completed forms along with payment to Babe Ruth Leagues, Inc.- Attn: Insurance, 1770 Brunswick Pike, P.O. Box 5000, Trenton, NJ 08638. Your coverage will be effective the day after postmark. You will then receive an insurance packet consisting of a certificate of insurance evidencing coverage, as well as claim forms and other pertinent information.
